Giveaway terms and conditions – NHS survey

Your chance to win a luxury hamper, worth £100.

All you have to do is be an existing customer of Practice Plan and take part in our NHS survey.

The following terms and conditions apply:

This free prize draw is being run by Practice Plan Limited of Cambrian Works, Gobowen Road, Oswestry, Shropshire SY11 1HS (Practice Plan, We or Us).

The prize draw is free to enter and is open to all NHS practitioners who are customers of Practice Plan and reside in the UK/IRE. Employees of any of the Wesleyan group of companies, their families and anyone professionally connected with the free prize draw are not eligible to enter.

No purchase is necessary, and you can only enter once. Multiple entries submitted by individuals using the same or a different email address will not be counted.

Entries should be submitted by fully completing the NHS survey on the relevant URL link provided to individuals.

The free prize draw opens on 1st December 2025 and closes at 17:00pm on 1st February 2026. Entries received after this time will not be counted.

There is one prize, and the winner will win 1 x luxury hamper worth £100.

We do not accept any responsibility for entries unsuccessfully completed due to any technical fault including, without limitation, where you submit incorrect contact details.

The winner of the free Prize Draw will be randomly selected from all surveys correctly submitted in accordance with the instructions and these terms and conditions within one week of the free prize draw closing date. The winner will be notified no later than 14 days following the free prize draw closing date. The winner will be contacted via the contact details they completed the survey with by telephone and/or email. If a winner does not respond to us within 14 days, then the winner’s prize will be forfeited, and we may select another winner in accordance with these conditions.

The winner will receive their prize by post within 1 month of the survey closing.

We reserve the right to substitute another prize of similar value. The prize is non-exchangeable, non-transferable and no cash alternative is offered.

The outcome of the free prize draw is final, and no correspondence shall be entered into.

This free prize draw is in no way sponsored, endorsed or administered by, or associated with any social media channels or suppliers of the prize.

None of the Wesleyan or Practice Plan Group or any of their agents or distributors will in any circumstances be responsible or liable to compensate the winner or accept any liability for any loss, damage, personal injury or death occurring as a result of participating in the free prize draw or taking up the prize except where it is caused by Wesleyan’s or Practice Plan’s negligence. Your statutory rights are not affected.

Participants are deemed to have accepted and agreed to be bound by these terms and conditions upon entry. We reserve the right to refuse entry or refuse to award the prize to anyone in breach of these terms and conditions.

We reserve the right to hold void, cancel, suspend or amend the promotion without notice due to reasons outside of our control.

By entering this free prize draw, you agree to your personal information (including any photographs) being used to process your entry and for participation in the free prize draw in accordance with these terms and conditions. Please be aware that personal data shared on social media will also be subject to that platform’s privacy notice so you should familiarise yourself with such terms.

Entrants hereby grant us a non-exclusive, perpetual, worldwide licence to use the entries (including any photographs) to promote Our brand (including on social media) and this free prize draw in the locations which We are based and you waive all moral rights in your submission for the benefit of Us.

The prize draw will be governed by English law and subject to the jurisdiction of the English courts.
